It Starts With the X-Ray

Before I touch anything, a panoramic X-ray shows me how each tooth is sitting: its angle, the shape of the roots, and how close it runs to the nerve in the lower jaw. With a full bony or sharply tilted tooth I will usually section it, which means dividing the tooth into two or three pieces so each piece lifts out through a smaller opening. Patients hear that and wince, but sectioning is the gentler option: less pressure on the bone, a smaller site, and faster healing than levering a whole tooth out in one piece. The Extraction, Step by Step

One simple extraction takes about 15 to 20 minutes. Four impacted teeth run closer to an hour. Either way, this is the path from the chair to the front desk.

1Consultation and X-ray

The panoramic X-ray maps every tooth, its roots, and the nearby nerve, so the plan is settled before we begin.

2Comfort and anesthesia

The area is numbed with local anesthetic. If you would rather sleep through it, sedation is ready.

3Gentle removal

The tooth is loosened and lifted out, sectioned into pieces first if it is impacted, for an easier exit.

4Cleaning and sutures

The socket is cleaned and dissolvable stitches go in when the gum needs help closing.

5Gauze and the clot

You bite on gauze to form the blood clot that protects the socket, which is the most important part of healing.

You are in normal company. A good share of my extraction patients tell me afterward that they dreaded it for weeks and then found the appointment unremarkable. Recovery and Aftercare

Most patients feel much better within three to four days. Swelling peaks around day two, then fades a little more each morning. These are the habits that make the biggest difference to how the week goes.

Bite firmly on gauze for the first hour to control bleeding and set the clot.
Ice the cheek, 20 minutes on and 20 off, through the first day to hold swelling down.
Rest with your head elevated and take it easy for 24 to 48 hours. Skip the workout.
Eat soft and cool, and chew on the opposite side of your mouth.
Take pain medication as directed, then start gentle warm salt-water rinses after the first 24 hours.
Keep brushing your other teeth, steering carefully around the surgical sites.
Protect the clot and you avoid dry socket. For the first few days, do not use straws, do not smoke, do not spit forcefully, and do not rinse hard. Any of those can pull the clot loose and leave bone exposed, which is the throbbing ache people call dry socket. If your pain suddenly gets worse three or four days out, call us. It is easily treated and I would much rather hear from you early.

There is no single right age, but the late teens and early twenties are usually the easiest window, because the roots are not fully formed yet and the bone is more forgiving.
